This will take you to Adobe’s website and enable you to download the Acrobat program to your computer.Click Here: New Patient Welcome Packet and Baby’s First Visit Form Your Welcome Packet has documents and forms that need to be reviewed and signed before your first appointment. To speed up the check in process, it is helpful if you fill out these forms before your appointment and bring them along with you. Included in your packet is a Release of Medical Information Form. Once completed, please give this to the clinic that you are transferring from so that your child’s records can be forwarded on to Goldstar Pediatrics. Copays and Deductibles When an insurance plan requires a copay, this is due at the time of the visit and is a contractual obligation that cannot be waived by the office. When a patient has deductible, we collect the estimated payment due at the time of the visit. Once insurance processes the claim, any remaining outstanding amount is then billed, or if there is a credit due, this will be refunded. Vaccine Policy At Goldstar Pediatrics, we believe in the safety and effectiveness of vaccines in protecting children against vaccine-preventable diseases and the complications they cause. We strongly support vaccination according to the CDC schedule for vaccination which is also approved by the American Academy of Pediatrics. We are unable to accommodate families that have made the decision not to vaccinate their children but are happy to help provide a list of other local pediatric offices that families may reach out to. Insurance Insurance eligibility will be verified before each visit. Below is a list of insurance plans we are contracted with. We continually update the list of accepted insurance plans. Late policy Arriving late to your appointment creates an inconvenience for the other patients scheduled to be seen after you. Patients arriving 15 minutes after their scheduled appointment time will be asked to re-schedule. Late arrivals for sick visits will be worked into the schedule, but will have to wait for the next open slot. Cancellation and No Show Policy We require one business day’s notice if unable to make it to an appointment. This allows us to open up the slot for another patient. We encourage you to give ample notice if you cannot make an appointment. Missed appointments without prior notice are considered a no-show. We understand that emergencies occur, so no fee is applied to the first occurrence. However, subsequent no-shows will attract a fee and eventual discharge of the family from the practice if multiple no-shows occur. Split Family Policy We believe divorce, separation, and custody agreements should not interfere with a child’s medical care.A parent requesting medical treatment is individually responsible for the payment of that visit’s medical bills.Goldstar Pediatrics is not a party to your divorce agreement; we will collect co-pays and deductibles from the attending parent and will hold the attending parent responsible for any other payments associated with that visit.“Joint Custody” means that each parent has equal access to the child’s medical record. Without a court order, we cannot restrict either parent from access to their child’s medical information.We will not call the other parent for consent prior to treatment, or to inform the non-present parent of the assessment and/or plan of care.We will discuss information pertinent to the child’s history and exam with the accompanying parent at the time of the visit; it’s the parents’ responsibility to communicate with each other. For this and other reasons, we recommend that both parents have access to the Patient Portal account, so each has access to their child’s medical records at all times.If a court order requires us to do otherwise, we will be happy to comply after we receive a copy of it, which we will enter in the child’s medical records.Should issues between parents become disruptive to our medical practice, we reserve the right to discharge a family from our care. Technology Our practice prides itself on efficiency through use of technology. You are encouraged to consult our website, register for and use our patient portal, and effectively use automated reminders for appointments and for routine care/immunizations that are due.
