Preparing for Your First Visit To make your first visit as smooth as possible, please bring the following items: Paperwork from the hospital, including hearing screen results and immunization records. Completed new patient forms including the Baby’s First Visit Form (you can download these from our website ahead of time). Any insurance information available (even if it’s only the parent’s details for now). Identification Card for parent. Before Your Baby Arrives A few weeks before delivery, call Goldstar Pediatrics to let us know you are expecting, and we’ll answer any questions you may have. Download our new patient forms from the website and complete them ahead of time. We recommend taking the Baby’s First Visit Form with you to the hospital and asking the nurses for help to fill it out. This is very important because sometimes it takes the several days or weeks for the hospital to send over the baby’s newborn records. After Delivery When your baby arrives, let the hospital staff know you’ve chosen Goldstar Pediatrics as your pediatrician. We’ll work closely with you to schedule your baby’s first appointment and ensure you feel confident and supported as you begin this exciting new chapter. It is best to call the office at the time of discharge so we can schedule your baby’s first appointment.
